Before your first visit, download and fill out the New Patient Intake forms by clicking on the buttons below. Please bring them with you to your initial visit.
For your comfort, please wear loose fitting clothing that can roll up to your elbows and knees.
We accept payment by credit card, check, Venmo, or cash at time of service. We are not in network for insurance, except for workman's comp. However, if you have out of network acupuncture coverage, we can provide you with documentation for you to submit to you health care insurance provider.
